AN IMPORTANT MESSAGE FROM EQUIFAX CANADA

New Billing Code feature.

Starting Monday, February 9th 1998, the following changes will be made to the "Consumer" application form :

• A new "Billing" field will be added to the bottom of the form to accommodate clients with multiple billing codes. If you have multiple billing codes, please select the appropriate code before submitting your request.
• The "Reference","Product" and "Billing" fields along with the "Submit" and "Reset" buttons will be moved from the top of the form to the bottom.
